[midPoint] Prefix namespace bindings may not be empty

...I forgot to add something related to second part of your mail: are 
your test & production environments really identical? What version?

What you see might be related to the exception mentioned. Generally I'd 
say that in the "production" case the "c" XML prefix is not defined in 
the XML object while serializing it, so it's added there. Nothing 
special. But it's really interesting that the other environment does not 
do that.
