[midPoint] CSV connector with single identifier

Hi,
we are on master, and there are different samples, I am afraid - look here:
https://github.com/Evolveum/midpoint/tree/master/samples

However, my coworker remembers that we needed to change icfs:uid for
icfs:name, since some midPoint version.
So, the sample may be outdated.

Try to do something like changing <ref>icfs:uid</ref>
to <ref>icfs:name</ref>
