[midPoint] Custom forms in self-registration page in v3.5

I am almost certain that the forms will not go into 3.5.1.

It's definitely not a bugfix. And it would need nontrivial effort to put 
it into 3.5.1 (e.g. because db migration scripts should be provided, and 
so on).
