[midPoint] Mandatory fields

Does this look to be correct, I kind of did it to see if it worked and it
seems to work fine, I was applying the same principal as above to custom
schema extensions,

   <item>
      <c:ref xmlns:bshp="http://www.bshp.edu/midpoint/custom/bshp
">c:extension/bshp:eduPersonAffiliation</c:ref>
      <limitations>
         <layer>presentation</layer>
         <minOccurs>1</minOccurs>
      </limitations>
   </item>
